What this website collects
This site itself doesn't have a contact form or a server-side database. The phone number and email address on our Contact page are plain tel: and mailto: links — clicking one opens your own phone or email app, and nothing you send that way passes through or is stored by this website.
Third-party services
A few things on this site are run by other companies, under their own privacy practices:
- Appointment booking — the "Book Your Appointment" links go to our patient portal at
healthrisemedical.cortico.ca, operated by Cortico. Any information you enter there is collected and stored by Cortico, not by this website. - Annual Block Program payments — the subscription links on our Annual Block Program page go to Helcim's hosted payment pages (
subscriptions.helcim.com). Payment and billing details are handled entirely by Helcim. - Map — the embedded map on our home and contact pages is provided by Google Maps, which may set its own cookies or collect usage data under Google's privacy policy.
Browser storage
This website does not use cookies or browser local storage of its own.
Your health information
This page covers this website's handling of information only. Health information shared with our clinic — through booking, in-office visits, or our patient portal — is handled by HealthRise Medical Centre as a health information custodian under Ontario's Personal Health Information Protection Act (PHIPA), separately from this website. If you have questions about how your health records are managed, please contact the clinic directly.
